Required Qualifications: Current NYS Registered Nurse License, Master's Degree in Nursing, NYS Certification as an Adult or Family Nurse Practitioner within one year of hire. Three years experience in nursing in an acute care setting.
Preferred Qualifications: Five years of current tertiary level care experience as a staff nurse or nurse practitioner. Current experience as a nurse practitioner providing anticoagulant management in an inpatient or outpatient setting that includes physical assessment, laboratory monitoring, patient education and monitoring for quality improvement. Proficiency in MS Office specifically Excel, Access and database maintenance.

Brief Description of Duties:
  • Diagnosis and treatment of outpatient Vascular clinical practice patients including pediatric and adult patients. Triage and assessment of patients by preforming history and physical examinations. Support of Vascular attendings.
  • Coordination and assessment of pre-hospital evaluation and follow up for admission, inpatient care and discharge planning.
  • Performing approve diagnostic and therapeutic procedures. Formulating treatment plans and reviewing lab results as well as consults in collaboration with attending physicians.
  • Initiation of referrals and consults to other health care providers and attending physicians for further definitiative therapy. Provide patient education and counseling and education of nursing staff regarding assessment and treatment of Vascular Surgery patients.
  • Participation in Research, Quality Improvement reviews, Infection Control etc. as dictated by the practice and the hospital.
  • Attend multi-disciplinary continuing education courses pertaining to medical/surgical care.

Special Notes: Full-time day 830am-5pm position. All Hospital positions are subject to changes in pass days and shifts as necessary. This position may require the wearing of respiratory protection which may prohibit the wearing of facial hair. Please note Stony Brook Medicine is a smoke free environment.
