Required Qualifications: RN Licensure in NY State. Bachelor degree or Associate Degree working on Bachelor degree. Five years experience in acute care hospital. Excellent communication and interpersonal skills. Understanding of discharge planning and barriers in the system to discharge. Computer proficient in Microsoft, Excel, Power Point. Leadership skills, ability to run meetings, rounds, etc. High level customer service. Weekday and weekend flexibility. Evening with some day rotation.
Preferred Qualifications: Master's Degree. Experience in Managed Care/Insurance Industry/MD Office. Experience in CM, UR, Quality, Risk or coding. Experience in Pediatrics, NICU, or Emergency Room. Certified in CM, UR, or Quality. Working knowledge of Milliman/Interqual criteria. PRI Certified. Recent experience in tertiary care hospital. Knowledge of regulatory requirements i.e. CMS, EMTALA, Managed Care, etc. Bilingual. Leadership experience.

Brief Description of Duties:
  • Screen patients for Level of Care including Acute In-patient observation and Out-patient.
  • Utilization review, Coordination of Care for all patients with the Interdisciplinary Team especially for delays and discharge.
  • Review one day stays for level of care.
  • Apply Condition 44 when needed for Medicare patients.
  • Provide retrospective review for short stay.
  • Communication with admitting, transferring hospitals and payers for authorization prior to transfer.
  • Chart documentation and charting including data management and statistics.
  • Promote a safe and timely patient flow from admission to discharge, including follow up for delays, discharges, tests, procedures etc.
  • Coordinate care for discharge planning including patient and family education for discharge with the educator and interdisciplinary team members and preparation of materials forms as needed.
  • Actively involved in performance improvement activities including program development, data collection and analysis.
  • Maintain a high level of customer service.
  • All unit coverage as indicated.
  • Other duties as assigned.

Special Notes: Shift: Full time day/evening shift. Hours: *Start: 7,8,9,10 am. End: 3,4,5,6 pm with two to four weekend days per month. All Hospital positions are subject to changes in pass days and shifts as necessary. This position may require the wearing of respiratory protection which may prohibit the wearing of facial hair.
