-Collecting supervisor hierarchy 1 - 3, minimum of 2.
Instructions and Examples:
Please complete the labor schedule for all changes except terminations. Current policy states that employees are required to sign and date the form when it is being completed for a resignation, retirement or an involuntary severance of employment. Beginning 2003, weekly hours for graduates are to be entered.
Please do not highlight or grey out cells.

-Payment for grad student tuition for grant proposals after 3/15/04
-Need at least two supervisors, primary supervisor, and secondary supervisor incase primary supervisor can not be reached.
