1. The Health Insurance Option Transfer Period Deadline For State Employees (Hospital & LISVH) Has Been Extended To January 5, 2009.

The 2009 state health insurance rates have been approved and were mailed by Albany to the home of NYSHIP enrollees recently. January 5, 2009 is the deadline to change from one health insurance plan to another and the deadline for employees to change from family coverage to individual or cancel coverage if they were unable to do so during the year due to the pre-tax contribution rules. To make a change, complete and return a NYS Health Insurance Transaction form (PS-404) to the Benefits Office before the deadline. If you are thinking about changing your health insurance plan, obtain a Choices for 2009 booklet, which summarizes the 2009 health insurance plans. The PS-404 form and Choices booklet may be obtained from the Benefits Office (45 Research Way, Suite 204, Tech Park, Zip=9212, from the Benefits display rack (HSC, Level 3, outside room 122), or online.
