Enterprise Rent-A-Car has just announced that they will now require the following documents when renting a vehicle and using a personal credit card as the form of payment. Renters must have the following:
A New York State Tax Exempt form, AC-946, when renting a vehicle in-state. This form is available at:

An authorization on State Agency letterhead advising that the traveler is renting for NYS business purposes only. Also, please include the dates of the rental in the letter.

2. Budget-Rent-A-Car
New York State has announced that it has entered into an agreement with Budget-Rent-A-Car for car rentals outside New York State. This agreement provides passenger vehicle rental services for all New York State employees when renting vehicles outside New York State. Rental pricing includes Loss Damage Waiver (LDW) insurance and unlimited mileage. Reservations can be made by calling Budget at 800-527-0700 or by visiting their website at www.budget.com. When booking, the Budget Corporate Discount (BCD) code of X930900 must be given. Reservations can also be made through Austin Travel, a State contracted travel agency. The keynote speaker is Paul Ginsburg, PhD, President - Center for Studying Health System Change. This workshop is designed to take you through the process of entering various types of procurement requisitions in the Lawson system including: Entering, changing, and deleting Stores and Vendor requisitions; Adding another page of items; Adding Comments to a requisition; Checking for approvals; Finding a purchase order number for a requisition; Inquiring on a purchase order; Finding receipts for a purchase order; Inquiring on requisitions; Inquiring on a budget; and Blanket Order releases. All Lawson users must attend this course to obtain a user ID & password. Have you ever wondered why some people do better than others when confronting difficult life situations? Depression affects over 15 million Americans each year. The good news is that it's treatable. This workshop will focus on ways to reduce your risk of becoming depressed. Workshop ofjbectives are: To understand the important role of exercise, nutrition and sleep in combating stress and depression; To recognize how our thoughts, feelings and behavior influence our ability to cope with life events; To identify signs and symptoms of clinical depression versus the blues.

The Educated Caregiver series was created to ease the burdens caregivers face and help them to provide quality care for their loved ones. 3. Long Island Offshore Wind Energy Presentation at the Marine Sciences Research Center
A 140 megawatt wind farm is proposed to be built three to six miles southwest of Robert Moses State Park. To learn about the economic, environmental, engineering, and public health facts and issues, as well as regulatory processes, the Marine Sciences Research Center will sponsor a special presentation by the Coalition of the Long Island Offshore Wind Initiative (LIOWI). LIOWI is a partnership of over thirty local and national groups working with the Long Island Power Authority on the Long Island Wind Project. Members of LIOWI will be available to answer questions after the presention. Wednesday, October 6, 7:00 p.m., Marine Sciences Research Center (South Campus), Endeavour Hall 120.

13. "The Purchase of Fruitfulness: Infertility and Assisted Conception in Middleton's A Chaste Maid in Cheapside"—a talk by Catherine Belling
Catherine Belling, Assistant Professor of Preventive Medicine, Associate Director of the Institute for Medicine in Contemporary Society, and author of articles in Literature and Medicine, Archives of Internal Medicine, and Academic Medicine among others, will deliver the second talk in the Feminist Campus Colloquia on Thursday, October 7, at 4:30 pm in the Humanities Institute, Melville Library, E4340.
The Feminist Campus Colloquia aims to bring together scholars from across campus interested in scholarship that attends to sex and gender. Speakers will present interdisciplinary work and scholarship that comes out of individual disciplines.

3. Celebrate National Physician Assistant Week and Promote Health Literacy
October 6th is a special day for the Physician Assistant profession. On this day in 1967, the first PAs graduated from Duke University. Today more than 50,000 PAs are providing essential medical and surgical services to people around the world. In commemoration of the beginning of the profession, many PAs host community activities and public information projects throughout October.
This year during National PA Week, Stony Brook PA students and Pre-PA Club will focus their efforts on the promotion of health literacy. We know that early literacy improves health literacy as adults. The students will sponsor a book drive from October 4-15th. Please bring your new or used books (all ages) to the collection centers in UH lobby, HSC level 2 (security desk) and Student Activities Center lobby. All books will be donated to hospitalized pediatric patients and to SCOPE educational services. Thank you.

1. University Art Gallery in Staller Center to Feature Four Faculty Artists
Mixed media artworks by the four most recent faculty additions to Stony Brook University's Department of Art will be exhibited at the University Art Gallery in the Staller Center for the Arts from Tuesday, September 14 through Saturday, October 16. The exhibition entitled "Recent Visions" features works by Stephanie Dinkins, Christa Erickson, Grady Gerbracht, and Nobuho Nagasawa that include a wide range of materials, concepts, and artistic practices. Many pieces incorporate video, sound, digital imagery, unusual materials, and computer controls. University Art Gallery hours are Tuesday through Friday, noon to 4:00 pm, and Saturday, 7:00 to 9:00 pm. The Gallery will also be open for one hour before and during intermission for some Staller Center performances. A closing reception honoring the artists will be held on Saturday, October 16 from 7:00 to 9:00 pm. Admission is free. For information, call 631-632-7240.

5. Stress Management Workshop
The Division of Wellness and Chronic Illness in the Department of Family Medicine will be presenting a series of four workshops on stress management.
These workshops focus on meditation as a core experience for stress management. The group is introduced to several forms of meditation including breathing techniques, the stress relaxation response, mindfulness meditation, walking meditations, heart-centered meditations and others.
Other aspects of stress management discussed are sound nutrition, movement, sleep practices, time management and prioritization, awareness of thought patterns and related emotions.
Workshop dates are Wednesday, October 20, November 3, 10, 17, 2004 between 5:30 and 6:45 pm at 33 Research Way, East Setauket, NY.
Lower your stress and register early for this popular workshop since seating is limited to fifteen participants. The fee for the four sessions is $80.00 payable at first session.

6. Defensive Driving Program Offered for UUP employees and their immediate family members.
A NYSUT sponsored Defensive Driving program is being offered on Monday and Tuesday, November 8 & 9, 2004 from 6:00 PM to 9:00 PM in the Wang Center Room 101, for all UUP employees and their immediate family members. Attendance at both classes is required.
Cost is $25 per person. Checks should be made payable to "NYSUT Benefit Trust" and delivered to the UUP office, 104 Old Chemistry, Z=3475. Office hours are Monday thru Friday, 9 am to 3 pm.
Participants are usually entitled to a 10% discount on their automotive insurance. Check with your provider. The certificate is good for three years of savings!

2. Alzheimer's Disease Trial At SUNY Stony Brook.
A Trial of Zocor (an anti-cholesterol medication) to slow the Progression of Alzheimer's Disease. All eligible participants with mild-moderate AD would remain on their current AD treatments, while participating in this trial.

3. How Do You Manage Your Stress During Pregnancy?
Research Study at the General Clinical Research Center, University Hospital.
If you are in the first or second trimester of pregnancy, you may be eligible for a research study that is looking at different ways to manage stress and cope during pregnancy.
Through this study you may learn relaxation techniques, coping skills and stress reduction techniques. These skills may be useful to you during and after your pregnancy.

4. Select - Selenium And Vitamin E Cancer Prevention Trial.
SELECT is a research study being conducted to see if prostate cancer can be prevented.
Men who participate in this study will take two capsules a day. Some men will take only selenium or vitamin E and some will take both. Others will take a placebo (sugar capsule).
Participants will visit their local study site twice a year for at least seven years, and no more than twelve years. About 32,000 men will take part in this study.
Men free of prostate cancer who are age 55 or older may be able to take part in this study. African-American men may join at age 50 or older.
Taking part in SELECT may not help you directly. But we hope that the information learned from this study will benefit your sons, grandsons, and future generations.

10. Did You Ever Wonder Why Some People Get Stressed Out More Easily Than Others?
Stress can affect the brain, heart, hormones, and mental performance.
We are looking for men and women, 18-50 years of age, to participate in a non-invasive study.
Participation in this protocol requires that volunteers be free on two consecutive weekdays including two hospital overnights
For example: Sunday night to Tuesday afternoon, or Tuesday night to Thursday afternoon
You will be free to pursue your normal activities after 4:15 pm, but you will need to be back for the overnight stay.
COMPENSATION: $200.00
