1. State and Research Foundation employee W-2 information

State employees - Your 2003 W-2 year end tax statement will not be distributed on campus this year. The Office of the State Comptroller in Albany will mail your W-2 directly to your home address.
Research Foundation employees - As in past years, your 2003 W-2 year end tax statement will be mailed directly to your home address.
By law, the State of New York and the Research Foundation are mandated to mail all W-2's prior to February 1.

2. ATTENTION all Notes 6 users: You must enable an archive setting.

We are moving ahead to our next step in the upgrade to Notes 6 - the server upgrade. If you are using the Notes 6 Client, once your home mail server is upgraded you will no longer be able to archive documents if you don't have an archive setting enabled. This is a very easy change that we recommend you do right now, even if your home mail server has not been upgraded yet.
To determine if you have the Notes 6 Client: From your INBOX click HELP and then click ABOUT NOTES. Look for the RELEASE number.
For instructions on how to enable the archive setting go to our Client Support website If you have any questions call the Client Support at 2-9800 or the UHMC Help Desk at 4-HELP.

5. SBU/BNL Van Route

The Office of the Vice President for Brookhaven Affairs has implemented a van route between Stony Brook University and Brookhaven National Lab. The van runs twice daily between the two institutions and is free of charge to students, faculty and staff.

6. The Stony Brook University Student Health Service Is Offering The Flumist Vaccine Free To Students, Faculty And Staff.

It's not too late to get your vaccine against influenza. While supplies last, the Student Health Service will be giving out FluMist Vaccine FREE to students, faculty and staff members of Stony Brook University. 1. Infection Control 2003 Training - JCAHO annual compliance - Last Chance

Infection Control 2003 training is required of all clinical employees of Stony Brook University Hospital and has been sent to all staff. People Soft Data sheets were to be returned to the Healthcare Epidemiology office. If you have attended the Nursing Recertification, CA or Support Services Recertification program, you have received Infection Control annual update training. You do not need to additionally participate in reading the training booklet.
For those who have not attended recertification, all completed data sheets should be returned as soon as possible (if not already done) to Healthcare Epidemiology, Hosp. L1-716, Zip=7018 via inter-hospital mail no later than January 31, 2004 in order to be processed and documented in a timely fashion for JCAHO annual compliance.

3. Notice to hospital (state) employees who enrolled in the Flex Spending Account Program for 2004

State employees who enrolled in the Flex Spending Account Program (Dependent Care Advantage Account/Health Care Spending Account) for 2004 had the first deduction taken from the January 14, 2004 paycheck, instead of the January 28, 2004 paycheck. As the contribution amount you selected was to be spread over 24 paychecks, (not come out of the first or last paycheck of 2004), you will not have a deduction on the last two paychecks of 2004.
This means your contribution amount for 2004 will be spread out over the first 24 paychecks of 2004. If you have any questions regarding your deductions for the Flex Spending Account Program please contact your Benefits Office or the Flex Spending Account Program at 1-800-358-7202 or visit the website www.flexspend.state.ny.us

4. Notice to M/C and UUP hospital employees who participated in the Sick Leave Exchange Program in 2003

Management Confidential (M/C) and United University Professions (UUP) employees who participated in the Sick Leave Exchange Program (SLEP) during the calendar year 2003 were supposed to, but did not have a SLEP credit applied to their health insurance premium on the December 31, 2003 paycheck. To correct this, Civil Service has notified us that the credit will be applied toward your health insurance premium on the January 28, 2004 paycheck.
At this time, there is no Sick Leave Exchange Program for the 2004 calendar year.

Through a series of Action Steps, you'll learn ways to organize your finances and make wise decisions about your financial future. None of the action steps require any prior expertise or experience with money. All that's required is your willingness to (1) set some goals, (2) collect and record information and (3) take one small step after another to accomplish them. Once you know where you want to go with your money (your goals) and where you are now (your records), you'll be ready to plan the future of your finances.

On Thursday, January 29th, the Department of Neurobiology and Behavior will host a Seminar given by Dr. Maria Diverse-Pierluissi, , of the Department of Pharmacology and Biological Chemistry at Mount Sinai School of Medicine. Dr. Diverse-Pierluissi's talk, "G Protein-Mediated Regulation of Spatial Distribution of Calcium Channels" will be held at 12 Noon in Room 038 of the Life Sciences Building.

SUNY Chancellor Robert King has just announced a new Chancellor's Award for Excellence in Faculty Service. 6. Film, Food, and Feminism Series presents If These Walls Could Talk I.

The Wo/Men's Center's Film, Food, and Feminism Series presents If These Walls Could Talk I. In honor of the 31st anniversary of Roe v. Wade, a screening and discussion of If These Walls Could Talk I will be shown during Campus Lifetime (Wednesday, 12:40 - 2) on January 28. The film presents vignettes of three different women's lives and experiences with abortion, over three different points in recent history. Home-cooked food will be served! 1. Habitat for Humanity, It's that time of year again when SPD begins our Habitat for Humanity fund-raising activities.

SPD is hosting on-going 50/50 drawings every two weeks beginning with the first drawing on January 16th. Drawings will be on Friday - the pot will be split - 50% going to the ticket holder and 50% going to the Habitat for Humanity Fund. Drawings will be held on 1/16, 1/30, 2/13, 2/27, 3/12, 3/26, 4/9, 4/23, 5/7, and 5/21.
Tickets are $1.00 each, 6 tickets for $5.00, and 13 tickets for $10.00. Tickets may be purchased from Marlene Brennan through each Thursday afternoon prior to the Friday drawing.

Kick-start your golf game with this golf-specific training program which will help you improve your game. This fitness program will focus on developing power, balance, and flexibility by incorporating exercises that will work on core strength and spinal stabilization. Exercises will mimic movements of game and include useful training aids like the physioball and resistance bands. Exercises will consist of yoga poses for balance and stability, pilates movements for limbering and flexibility, and strength training exercises for power and strength.

2. Defensive Driving Program Offered for UUP Employees and Their Immediate Family Members

A NYSUT sponsored Defensive Driving program is being offered on Saturday, February 7, 2004 from 10:00 AM to 4:00 PM in the SAC Room 303, for all UUP employees and their immediate family members.
Cost is $25 per person. Checks should be made payable to "NYSUT Benefit Trust" and delivered to the UUP office, 104 Old Chemistry, Z=3475. Office hours are Monday thru Friday, 9 am to 3 pm.

2. Are You Depressed? We Can Help!

The Department of Psychology at Stony Brook University is conducting up to a 6 month clinical trial involving medication with all participants and the addition of psychotherapy with some. Must be willing to travel up to two times a week for up to 3 months to the site and be available late afternoons/early evenings. If you have been feeling depressed (sad, down) most of the time for at least two years, you may be eligible to participate. All evaluations and treatments will be provided free of charge.
