Electronic Reserve Request Form
The Electronic Course Reserve Request Form is for teaching assistants and faculty wishing to place supplemental reading material on Reserve. You must create or already have a course in Blackboard (http://blackboard.sunysb.edu/), if you are going to use E-Reserves. Access to the documents in E-Reserve will be limited to faculty and students registered in each course.
Due to copyright restrictions, library staff can only scan one chapter from a book, an article from a journal, a short story, essay or short poem, a chart, graph, diagram, drawing, cartoon or picture from a book, periodical or newspaper. Items marked with a "*" are required fields. Prior to issuing reading assignments, please confirm that your request has been processed in Blackboard. Thank you.
