Electronic Reserve Request Form
The Electronic Course Reserve Request Form is for teaching assistants and faculty wishing to place supplemental reading material on Reserve. You must create or already have a course in Blackboard (
http://blackboard.stonybrook.edu/), if you are going to use E-Reserves. Access to the documents in E-Reserve will be limited to faculty and students registered in each course.
Due to copyright restrictions, library staff will scan only ONE chapter from a book, an article from a journal, a short story, essay or short poem, a chart, graph, diagram, drawing, cartoon or picture from a book, periodical or newspaper. Items marked with an "*" are required fields. Requests are processed as received. In order to accommodate many requests in the beginning of the semester in a timely manner for all, please indicate the date you will need each item to be available on Blackboard. Prior to issuing reading assignments, please confirm that your request has been processed in Blackboard.

Adding a Library as a Course Builder. In order for library staff to post scanned items to your Blackboard course, you will need to give course builder privileges to the appropriate library.
