The Course Reserve Request Form is for teaching assistants and faculty wishing to place supplemental reading material on Reserve. This form may only be used for books, CDs, cassettes, and Audio-Visual material. Items marked with a "*" are required fields. Prior to issuing reading assignments, please confirm that your request has been processed for Reserve in the appropriate library.

Please be aware that due to severe budget limitations, it may not be possible to purchase multiple copies, without authorization by a Library Subject Selector for your area. You may be contacted by the subject selector if he/she has a concern with your request. If possible please consider reducing the number of copies you require based on this information.
